Additionally, we can provide a free, 120-day supply of medicine to qualifying patients through our Novo Nordisk Patient Assistance Program.

To help make treatment more affordable, encourage your patients to sign up for the Novo Nordisk Instant Savings Card, which allows patients to:
Pay no more than $25 for many Novo Nordisk products and each refill for up to 2 years*
Pay no more than $20 for select other Novo Nordisk products and each refill for up to 2 years*
Be eligible to receive 1 FREE box of Novo Nordisk needles*
All you have to do is:
Direct your patients to sign up at Cornerstones4Care.com to receive a Novo Nordisk Instant Savings Card so they can take full advantage of all the savings. Patients can activate their card at NovoNordiskSavingsCard.com or by calling 1-855-292-5966 OR
Obtain a supply of Novo Nordisk Instant Savings Cards by contacting your Novo Nordisk Sales Representative.
There is a maximum of savings of $100 per prescription for each analog prescription. Should you or your patients have questions regarding setup, claim transmission, patient eligibility, or other issues, call the Novo Nordisk Instant Savings Card Program at 1-855-292-5966 from 8 AM – 8 PM EST Monday - Friday.

Novo Nordisk Patient Assistance Program
The Novo Nordisk Patient Assistance Program (PAP) is based on our commitment to people living with diabetes and on our philosophy known as the Novo Nordisk Triple Bottom Line. The Novo Nordisk PAP provides free diabetes medicine to those who qualify.
If approved, a free 120-day supply of the requested medicine or device will be shipped to the prescribing health care provider's office to be picked up at the patient’s convenience. Novo Nordisk will send the health care provider a reorder reminder every 90 days.

Gather proof of income
Bring the application and proof of income to your office to complete the Health Care Practitioner section
